Physio Nation is the trading name of Anne Keng, sole trader. Anne Keng is a registered healthcare professional providing physiotherapy services at The Hidden Gym. The Hidden Gym is a fitness facility located at 28 Magdalen Street, London, SE1 2EN. For the purposes of processing your personal data, Anne Keng as an individual health practitioner is the controller and can be contacted directly via email at akengphysio@gmail.com.
What Information Is Collected?
In order to provide you with Physiotherapy services, the below information is collected when you first book an appointment:
personal details such as name, address, date of birth, and mobile number
emergency contact details
relevant medical history and health data
insurance details (if applicable)
During your appointment, your health practitioner will also ask for information relating to your concern, general health, and lifestyle factors to determine the most appropriate management plan for you. As part of legal requirements, clinicians must document each physiotherapy session in clinical notes.
How Is Information Stored?
All data collected will be stored electronically through a secure cloud management system. All accounts will be password protected and the computer used has an automatic time-lock to ensure further security. No sensitive information is stored directly on laptops. We also do not store any payment details; all payments are collected by protected third parties.
Communication with you will be provided by email in relation to the services provided. This may include information such as booking confirmations, reminders, exercise programs, and invoices.

We use your information to provide safe and effective care, including:
Facilitating referrals to other health professionals
Keeping you informed about any changes or updates to the clinic or services

How Long Will Your Data Be Kept For?

You have a right for any corrections to be made to your personal data which you believe is inaccurate or incomplete.
You have a right to request the erasure of your data.
You have a right to request your data to stop being stored or used.
